University of Connecticut Data Breach (2012)

an employee responded to a phishing e-mail during the week of April 16, 2012. In doing so the employee provided access to a file that contained payroll related information for individuals were employed with the University dining services the apartment during 2011. This error was discovered on April 23, 2012.
