Thomas H. Boyd Memorial Hospital Data Breach (2015)

The hospital had leased a facility in Illinois to store records. The owner of the facility sold the building and did not inform the hospital. 7500 current and/or former patients of the Hospital were located in the building. The records contained personal information of the patients, including but not limited to patient names, dates of birth, social security numbers, addresses, account numbers, diagnoses, disability codes and treatment information.
