State of North Carolina Data Breach (2013)

CSC is a contractor for the State of North Carolina. In the course of performing services for the State, we put information from the Medicare Exclusion Database on a thumb drive. This information included your name, Social Security Number (SSN), federal tax Employer Identification Number (EIN), and date of birth. It also included other information from the database that is publicly available. In early March, we discovered the loss of this thumb drive in CSC facilities in Raleigh, North Carolina. Although not discovered until March, the thumb drive containing this information is believed to have been lost in late February.
