Owner-Operator Independent Drivers Association Data Breach (2012)

An unauthorized individual gained access to some coded online forms stored on a remote server housed with a third party ISP. The individual changed certain informatino on the forms which caused the forms to be sent to this individual as a blind copy email. The breach may have exposed customer's name, CCN, expiration dates, phone numbers, email addresses, DOB and credit/debit card service codes.
