Florida Agency for Health Care Administration Data Breach (2012)

Florida’s Agency for Health Care Administration reports that a breach involving its business associate, DentaQuest of Florida affected 1,892 patients. This was a printing error on the part of OnTouchPoint CCI, DentaQuest's print vendor. A programming error resulted in member ID cards being mailed to the wrong addresses.
