CVS Health Data Breach (2017)

On or about Jan. 11, an unknown individual(s) stole hard copy prescriptions from the pharmacy drop off counter area. It was determined that limited patient information such as name, date of birth and address were taken. It is not believed that obtaining access to this patient information was the primary purpose of the theft. No other data or information was accessed by the individual(s) responsible for the theft. CVS Health has sent a notice to patients whose personal information may have been accessed when their prescriptions were stolen.
