California Department of Public Health Data Breach (2012)

After conducting a survey at the Bakersfield Memorial Hospital, an L&C employee mistakenly left the survey binder in an unattended vehicle. Later, the vehicle was broken into and the survey binder was stolen. The information exposed includes name, DOB, medications, room number and medical record number.
