Bob Ward & Sons, Inc. Data Breach (2012)

E-commerce site is hacked, exposing name, address and credit card info of customers of www.bobwards.com. Discover card notified of CPP. Oddly, the letter indicates they were notified before the first initial compromise, which doesn't make much sense. They do state that past 5/31/2012 the data was no longer encrypted.
