Alicare, Inc. Data Breach (2012)

on January 13, 2012, Alicare, Inc., third-party administrator of the national retirement fund discovered a potential information security incident involving certain personal information of yours that was inadvertently provided to a mailing service imprinted on the mailing label affixed to a letter sent to you. The mailing label generated by the vendor inadvertently included the Social Security number of the participant the letter was addressed. It included your Social Security number along with your name and mailing address.
